When applying for the Award, the following must be followed:

1. Download the form designated for this purpose in English from the official website of the Ministry of Health. The form should be saved as Word document on the candidate's desktop and then filled out in English.
2. The completed form, supported by documents required for each category of the Award, should be sent to the office of the Minister of Health via the following e-mail

3. Or to be handed on a USB Flash Drive to Mr. Mahmoud Ahmed Isa, Program Coordinator in the Office of the Minister of Health, at the Ministry building in Juffair including all the attachments, personal information, and contact numbers for each candidate.
